DARIER'S DISEASE
Clinical Presentation
Vesicular Darier's (37654551)
-
Vesicular Darier's disease is a rare variant of the disorder where patients develop numerous vesicles and bullae concurrently or independent of the more typical lesions found in Darier's disease.
Treatment
Type II Cytokine Inhibitors
-
Inhibition of type II cytokines like IL-4 and IL-13 have been effective in case reports. 39046749
XYLAZINE-ASSOCIATED WOUNDS 39535749
Wound Characteristics
-
Most wounds (90%) were located on the extremities, with 69% involving extensor surfaces (e.g., posterior upper or anterior lower extremities).
-
Common wound bed features included devitalized tissue (60%), such as eschar or slough, particularly in larger or chronic wounds.
-
Wounds varied in size: 20% were small (<1 cm²), 56% medium (1 cm² to hand-sized), and 24% large (larger than hand-sized).
-
Chronic wounds (present for >3 months) were more likely to be large and have necrotic wound beds, compared to acute wounds (<1 month).
Etiology
-
The majority of wounds (87%) were linked to injection drug use, with xylazine exposure exacerbating tissue injury due to its vasoconstrictive and cytotoxic effects.
Infection and Complications
-
Signs of infection included erythema (58%) and purulence (25%), with some wounds showing exposed deep structures (e.g., bone, tendon).
-
Cultures revealed Staphylococcus aureus (including methicillin-resistant strains), group A Streptococcus, and other pathogens in infected wounds.
